A little preparation can make the appointment more useful and less stressful.

Before your primary care visit, take a few minutes to gather what you need and write down your questions. Bring this checklist with you.

Before your appointment

Confirm the date, time, and office location
Bring your insurance card and photo ID
Bring a list of current medications, vitamins, and supplements
Write down any allergies or medication reactions
Note your main reason for the visit
Write down symptoms, when they started, and any changes
Track things like pain, sleep, appetite, mood, blood pressure, or blood sugar if relevant
Bring records from urgent care, specialists, or recent hospital visits if you have them
Make a list of questions or concerns so you do not forget them
Bring your pharmacy name and location

Topics to be ready to discuss

New symptoms or health concerns
Ongoing conditions like diabetes, high blood pressure, asthma, or chronic pain
Medication refills
Side effects or problems with current medications
Preventive care needs like vaccines, screenings, and annual labs
Mental health concerns such as anxiety, depression, stress, or sleep problems
Changes in weight, energy, appetite, or daily functioning

Start with your biggest concern first
Be clear about what has changed since your last visit
Tell your doctor what treatments or remedies you have already tried
Mention any specialist visits, ER visits, or urgent care trips
Ask whether you are due for routine screenings or vaccines
Ask if you need lab work or follow-up testing
Ask what lifestyle changes may help
Ask for clear next steps and timing

What do you think is going on?
Is this something temporary or ongoing?
Do I need labs, imaging, or a referral?
Are my medications still the right fit?
Am I due for any vaccines or screenings?
What should I work on before my next visit?
When should I come back, and what should I watch for in between?
